• PharmD degree or equivalent experience
• Eligibility for Kentucky pharmacy licensure
  • PGY2 Infectious Diseases resident will also be required to obtain Ohio pharmacy license

• Three letters of recommendation
• Additional Requirement PGY-2 Candidates: Completion or current enrollment in PGY-1 program
