Community Health Needs Assessments

The surveys included questions about access to primary care, specialty physicians, health insurance coverage and use of emergency and urgent care treatment facilities. Participants were asked to identify the top five health issues in their communities and the challenges individuals face in accessing healthcare services.

Following the survey, a Community Health Improvement Plan (CHIP) was created.
